This is an online, fillable request form for both on and off campus recital requests. Submission of this form does not guarantee approval, and incomplete submissions will not be accepted. Once reviewed by the MEIS office, you will receive an email with approval confirmation and/or follow-up questions.

All Recitals:

• This form is due at least ONE WEEK PRIOR to the Recital PREVIEW.
• You are responsible for correspondence with your applied instructor regarding recital previews, and recital performances. Your applied lesson instructor must be present at both.
• You must coordinate with your applied lesson instructor to schedule and perform a recital preview no less than 2 - 4 weeks in advance of your recital performance. We recommend you start the recital planning process at the beginning of the semester.
• Recital previews and performances are an essential standard for all Performance & Singer Songwriter students. Please refer to our MEIS Student Handbook for consequences of failing your recital preview, and take note of our Student Code of Conduct to further understand your roles and responsibilities as an MEIS student.
